Subjective:- Sore bottom with discharge, smell, and pain on sitting- Discharge intermittent, strong malodorous smell- Painful to touch, requires muzzle for examination- Restless, poor sleep last night due to discomfort- Difficulty sitting down- Previous anal gland problems approximately 4-6 years ago, treated successfully- History of skin allergies, resolved with dietary modification (removed beef with bones)- Currently on home-cooked diet, recently introduced lightly cooked chicken wings with bones- No current defecation difficultiesObjective:- Weight: 46.2kg- Ambulation nad- Attempted PEx but limited, growling and trying to biteDiscussed sedation with O who consented.SC injection of 0.2mg/kg butorphanol + 0.02mg/kg medetomidine 6:40pm.Provided profound sedation within 20 mins.Rectal exam nad, small amount of AG material expressed from L AG, R empty. Normal stool.8x8x2mm raised area of thickened tissue Left lateral tail few cm from tail base, pin prick dark spot middle of this area.Spinal and abdominal palpation nad though sedated.Heavy tartar, moderate gingivitis.Reversed with atipamezole.Assessment: Possible trauma to tail base (insect bite, plant thorn) coinciding with AG release as cannot find other abnormalities on sedated exam and not possible to examine awake.G3/4 periodontal dz.Plan:Meloxicam SID 0.1mg/kg three days and assess response.Discuss dental treatment with regular vet.
