Concern: recheck for urinary issues. 

History.  
- Still not urinating normally in past week 
- Small and less frequent urination 
- Hasn't weed this morning
- Buprenorphine seemed to help 
- Eating and drinking less - but still is eating food. 
- Indoors cat 
- No other animals at home 
- Diet: Hills Adult indoor kibble 

Meds: 
- Buprenorphine 0.3ml Q12hr as needed 
- Prednisolone 0.4ml EOD 

Examine: 
Mentation: BAR
BCS:                    8       /9   
MM: Pink          
CRT: 1-2 seconds
HR:                 160            beats/min
RR:               30              breaths/min
Temperature:      38.7        degrees Celsius

Ocular: No abnormalities noted on cursory examination of both eyes.
Oral/Dental stage: Moderate gingivitis and calculus observed. Gx/4 dental disease. 
Ears: No abnormalities noted on cursory examination of both ears.
Cardiovascular: G2/6 heart murmur 
Respiratory: Respiratory rate and effort considered normal for the patient.
Gastrointestinal: No pain on abdominal palpation. No masses or abnormalities palpated.
Integumentary: lesions self induced on belly.
Musculoskeletal: Normal gait and strength noted. No evidence of lameness.
Urogenital: No significant abnormalities noted during routine physical examination.
Neurological: No evidence of neurological impairment or altered mentation. Detailed neurological examination not performed today.
Lymph nodes: Within normal limits for mandibular, prescapular and popliteal examination.

Problem List: 
dysuria.

Patient Assessment: clinically stable today, urinalysis to rule out UTI/crystals today, suspect UTI. Able to be monitored at home.

Diagnostics performed:
- Cystocentesis: sent off for culture and urinalysis 

Treatment: Nil 

Client Communication & Plan: 
- Continue on pain medication at home 
- Will relay urinalysis to O - any vet to call 
- If cat cant not wee at all them present to vet/emergency. 
- If cat not eating then call vet asap - can dispense appetite stimulants / will need RV.
