07/03/2026 REVISIT REASON: Skin starting to flare up. PROGRESS:Has lasted longer than usual, but has become itchy for past week. O feeding fish only diet. Using malaseb (feet) and oatmeal (body) shampoos. EXAM:Only mild erythema feet and perianal. Otherwise well. Chest sounds good. skin only mildly affected at present. ASSESSMENT: Doing well PLAN: Vet visit in 6m. OK for nurse to give until then. May get less injections with the change of the seasons.

REVISIT REASON: cytopoint PROGRESS: History: - went well with last cytopoint. getting itchy again and really licking at LF paw - o bathes paws with malaseb shampoo and when she really licks at an area/interdigital cyst o puts bepanthen on it and cleans with betadine solution. o reports this usually settles quickly - o would like to try the 40mg dose of cytopoint this time EXAM: ventrum good, paws flaring, palmar interdigial dermatitis mild/moderate, dorsal front paws moderate with large cyst on medial edge of toe 3 on LF - has early lick granuloma on it, some smaller interdigital cysts, dorsal interdigital spaces on hid paws has mild erythema ASSESSMENT: allergy flare - need to do cytopoints regularly to avoid flares. monitor cyst on LF toe 3 - may need AB's PLAN: cytopoint 40mg (batch 806985, exp 11/2026) NEXT RECHECK NEEDED: 4-6 weeks for repeat cytopoint - 40mg dose. can be with nurse for next 4 injections as long as going well and within this time frame
