Theng etting to point of strianing and nothing coming out - Was improving then went back to black - Was on ABs last week EDUD: All normal V+/D+/Coughing: None Pre-existing conditions: None Current medications: None Previous adverse reactions (vacc/meds): None Examine: Mentation: BAR Body condition score: 3.5/5 MM: P/M CRT: <2s HR: 100 w/ no murmur or arrythmia auscultated RR: WNL w/ no adventitious sounds auscultated LN: Submandibular, prescapular, and popliteal WNL Temperature: NE ---------------- Oral/Dental: No malocclusion noted. Mild dental disease Gastrointestinal: Comfortable abdomen, with no pain noted on palpation. No abnormalities felt. Integumentary & lumps (measurement, location): No lesions noted. No umbilical hernia seen or palpated. Neurological: No obvious neurological deficits noted. Full examination not performed ---------------- Problem List/Assessment: - D+ with melena A: Advised can provide gastroprotectant and prokolin to manage condition. This does not provide a diagnosis of cause but can resolve the signs. Discussed if worsening signs or continues then needs further work-up (bloods/US). O happy with this plan. Treatment: - Pro-kolin 5mL BID - Bland diet - Sucrulfate 500mg BID Plan/Follow-up: - RV INI
