- Cardiovascular: Heart sounds normal
- Integument: Small superficial wound on paw pad; appears grazed; no foreign body visible on probing; Rocky weight-bearing on paw

Assessment:
- Problem list:
1. Superficial paw pad wound (graze)

Plan:
- No cleaning required
- Monitor 24-48 hours
- Return if not improving for sedation and exploration
- Should heal naturally if nothing embedded

Communications:
- Discussed wound appears superficial with no visible foreign body
- Advised to monitor for 24-48 hours; if still licking or not improving, return for sedation and exploration
- No need to restrict walking on street
- Owner to check carpet at home for potential hazards
