History: 
O says that Pedro has had a skin issue for the past 2-3 months. 
Has noticed multiple areas of hair-loss and scabbiness over the dorsum, extending over the sides of his body. 
O says that he is very itchy and that he has noticed scabbiness and clumps of hair falling off. 

EDVD: normal 
Current medications: Previcox, Gabapentin 
Proph: UTD 

Examine: Very reactive today, only distance exam 
Integumentary & lumps (measurement, location):
- Multifocal areas of erythema, alopecia and scabbiness over dorsum and 
- Woods lamp negative. 

Problem List/Assessment: 
Pruritic with Suspected Pyoderma 

Plan: 
O will ensure he is UTD with prophylaxis. 
O to start with antihistamines. No option to use pred today as currently on previcox. 
Dispensed 2-week course of cephalexin. May required further course. 
Also advised using medicated shampoo 2-3x weekly. 
INI will need to do impression smear.
