Over the last couple of months O has noticed generally a little slower, less keen to jump onto furniture and intermittently holding up back leg (not sure which) - generally mobility still good but O just wanting a check. 
No hx of any changes in exercise of injury. 
Anxious dog esp at vet now since dental
otherwise well in himself - edud fine, no v/d/c/s

Diet: RC calm diet
Current medications:nil

Physical examination:
BAR/heady shy/anxious. Difficult to examine. Leaping up onto back legs in cons. 
HR:difficult to ascultate as panting heavily and unable to stop this completely - approx 120bpm, no audible murmur etc but difficult to hear

MM:pink, moist, <2
Temp: not taken
BCS: 7/9

Eyes: nad
Ears: nad
Nose: nad
Oral cavity: unable to examine due to behaviour - est g 1-2/4 from distance exam
LN: nad
CV: No audible murmur or arrhythmia. Strong and synchronous pulses
Resp; nad
Abdo: nad
Urogen: nad
Gait: Normal, leaping up on hindlimbs in cons. 
Msk: Good ROM neck, limbs inc hips. No crepitus or reduced ROM/pain. digits all wnl
Neuro: nad
Integ: nad

Assessment:
some clinical signs of decreased ROM at home and reluctance to jump but clinically today wnl. 
Dog very anxious so limited exam

Plan:
advised monitor and revisit ini/worse for repeat exam. Advised not to allow high intensity exercise for now in case soft tissue strain etc. 
Advised FF meds to help relax prior to next vet visit.
