06/03/2026 PPS certificate in office pu box please give at next app puppy vacc 06/03/2026 O came home about 20mins ago and noticed bernies eyes were puffy. came straight here and on the way in car vomited. not sure whats happened e: bar mm pm crt <2 hr 140 nad rr 20 nad temp 39.2 generalised angeodema over the muzzle and periocular region. no bee stingers found. a: appears bright and well but has facial swelling likely due to allergic reaction from insect sting or bite. advised admitting to monitor for the next 30-60min and giving niramine, dexason and maropitant as vomited. O happy with plan. Discharge with Anna as I have finished for today tx: 0.5ml dexason SC 0.5ml niramine SC 0.76ml vetemex SC

rx: ran into stairs and now limping. hx: only happen 7 pm, come straight down. was holding LF up off the ground and couldnt wait bear e: BAR mm pm crt <2 m/s g2/6 LF lameness when walking. weightbearing normally when standing. comfortable on manipulation/palpation of all joints. no swellings or obvious fractures/disclocations a: suspect mild soft tissue injury/ contusion. already improved in the last 45minutes since happening. recc once of nsaid injection and monitor. recheck tomorrow ini/worsens tx: 0.24ml metacam SC 6.1kg

Reason: History: owned 2 weeks from SA breeder has settled in well, no longer crying at night toilet training going ok normal stools O has owned labradors in past, both died relatively young and required specialist treatment so got this pup insured early Environment: Diet: Blackhawk puppy Preventatives: previous dog was on advocate plus a GI wormer q 3 months Medications: Examination : Demeanour: BAR BS= 5 / 9 DS= 0/ 4 normal deciduous dentition HR=normal rate and rhythm no murmur RR= no effort no abN noise MM= pink, CRT, 2 secs soft abdomen, NAD normal ROM all 4 limbs, very good exetnsion both hips and no discomfort on tibial thrust good coat, clean eyes and ears no hrnias Lab: Assessment: healthy pup ok for vax Treatment: protech C3 + intranasal PI/BB Plan: boodter c3 in 4 weeks
