<div><strong>History:<br></strong>New client / patient. No major health concerns in the last 4 years. <br>Only pet at home. <br>Has an itchy spot on his head which he has scratching at quite a lot. <br>O has done a blue light check of the spot on the head at home, has done a lot of home reading and wanted a skin scrape today. <br>No lameness concerns EVER, but one time Sparkles looked back at a vet when they extended his hips so "hip concerns" were raised.  O has since taken Sparkles to specialist at Southpaws and examination was NAD but advised if want to go further then it is GA for xray of hips.  This has not been done.<br>Also wants to go to specialist ophthalmologist just to check eyes even though no concerns.<br>Brushes teeth almost daily and showed me photo of three different types of pet toothbrush - wants to book in for dental clean, to probe his teeth etc.<br>O said from her country of origin it is the norm to do full physical, blood test, dental, dental xrays, echo etc etc on an annual basis.<br>Diet: Is only on wet food, sometimes homemade balance, and freeze dried food<br>Parasite prevention: Bravecto chew only in summer, NUTD for other parasites.  Used to protect for HW but was told by one vet unnecessary so has stopped<br>Vacc:  card brought in.  Had triennial nobivac Flex DHP on Feb 2023 with Nobivac BB.  Missed vacc in 2024, and in 2025 Feb had Protech Pi2 and Bronchishield</div>

<p><strong>Exam:</strong><br>BAR, calm boy, really lovely dog<br>Mm pink, CRT &lt;2s<br>Dental comments if any: Tiny spot of tartar only in the groove of the upper carnassial teeth - and UV light showed only one bit of pink at those teeth. Other teeth look great.<br>Eyes – NAD<br>Ears – NAD<br>Skin – 3 small scabs on top of head near area between eyes - old scabs by appearance, and underneath the skin is also healing but still a bit pink.  O very concerned and requested skin scrape but advised tape prep will suffice.<br>Tape prep performed - and nothing to see.  O found another small scab at back of head - tiny little scab and skin underneath a bit more pink.<br>Rest of skin NAD<br>Lymph nodes:  NAD<br>Cardiovascular auscultation quality - 112, normal rhythm, no murmur<br>Thoracic auscultation - chest clear<br>Abdominal palpation –  NAD<br>Urogenital system –  NAD<br>Musculoskeletal  – NAD on palpation.  At one point when talking O had both of Sparkles' hindlimbs in her hand and was extending them while gently bouncing the hips and he reacted not one jot.<br>Neuro: NAD<br>BCS 4/9<br>MC scanned &amp; recorded microchip<br><br><strong>In house diagnostics:</strong><br>In house tape impression of scab of skin under scabs on head<br><br><strong>Assessment:</strong> <br>Very mild irritation at two spots on head<br>Very very mild dental tartar on grooves of upper carnassial teeth<br>Healthy dog otherwise<br><br><strong>Treatment: </strong><br>Neocort cream should suffice for the two areas of skin of concern on head<br>Advised dental probe, S&amp;P can be performed but unnecessary at this stage.  If decide to pursue we can xray hips under GA at same time.<br>Assured O she is doing well looking after Sparkles but I have no major concerns at the moment.<br><br>Due C5 but O would like to wait until she is back from long weekend.  We can reassess to see how his skin is at same time.  O will research on HW and get back to us.....<br><br><strong>Plan:  <br></strong>1. Encouraged switching to a toothbrush when teeth brushing, concentrating more on the back teeth</p>
