HISTORYPresenting for: Teddy, a 7-year-old male desexed Cavalier Kind Charles spaniel, presented for excessive urination at night and polydipsia, fever and general lethargy.Diet: Increased hunger reported alongside increased thirst. However food has dropped off today and not eating even treats when offered.Drinking/Urination: Excessive thirst (polydipsia) and excessive urination (polyuria), particularly at night started last nigth but not todayCurrent Meds: NilOther: Small amount of discharge noted in left eye. Needs Dental extraction of Left Upper premolar and general clean of G1 Dental Disease.EXAMMentation: Reasonable spiritsWt: 5.6 kgT: 40.0°CHR: 160 bpmR: Increased respiratory rate, pantingEyes: Small amount of discharge in left eyeOral: Needs extraction of left upper premolar, dental disease presentAbdominal: Slightly tense abdomenRectal: Very empty rectum but moistDiagnostics: Blood glucose 4.4 mmol/L (not diabetic). Urine specific gravity 1.047 - ASSESSMENTPolyuria and polydipsia - diabetes mellitus ruled out based on normal blood glucose. Patient appears slightly dehydrated.Dental disease - left upper premolar requires extraction.Pyrexia - temperature elevated at 40.0°C.PLANAdditional Discussion: - - Monitor hydration status given slightly concentrated urine and clinical signs- Investigate underlying cause of Polyuria/polydipsia given normal glucose levels      - Owner has given consent for blood work to be completed.LABRATORY-  Urine Spec Grav 1.047-  Urine moderate/Dark Yellow in colour clear-  Normal potency - spot Glucose ( Owner requested 4.6nmol/L) NormalTREATMENT- IV Maropitant 1.10pm 0.56mls- IM Methadone 0.12mls IM 1.10pm for cranial abdominal pain.Assessment: Heart and lungs =OK  No increased trach sensitivity Hunched back and some back pain on palp 2.45pm T= 38.7  No drinking in hospital Lnn  all feel normal Rectum empty  expressed anal gl Laboratory: Bloods reveal neutrophilia    Kidney and liver Fx is normal as is amylase and lipase Treatment: Meloxicam 0.2ml sc Metrogyl 100mg bid 5dd Metered water intake  Chicken and rice elevated food and water bowls Plan: "O" to call Monday    Vital Signs    Weight: 5.6;       MMColour: pink;       Temperature: 40;       HeartRate: 160;       BodyScore: 2;       DentalGrade: 1;       RespirationRate: panting;       PainScore: 2/5;       CRT: 2 sec;
