Yesterday was mostly fore paws.<br>When zenrelia was dispensed the itchiness seemed to improve a lot, however when the tablets wore off the itchiness returned. While on the tablets there was still a small amount of itchiness, however no where near as itchy as without the tablets. <br>No known triggers, or wandering jew. The front paws are worse than the back, o has seen redness on all 4 paws. <br>Diet:<br>Preventatives: UTD<br>Vaccinations: UTD<br>Current medications : none <br><br><strong>Exam:</strong><br><br>BAR,<br>Mm pink, CRT <2s<br>Dental comments if any: 1, dental staining, could well do with a S&P - booked in for August to take advantage of dental month<br>Eyes – NAD<br>Ears – NAD<br>Skin – minimal redness on paws today and no evidence of infection. However, is pruritic at home.<br>Lymph nodes: NAD<br>Cardiovascular auscultation quality - HR 140, normal rhythm , no murmur<br>Thoracic auscultation - chest clear<br>Abdominal palpation – NAD<br>Urogenital system – NAD<br>Musculoskeletal – NAD<br>Neuro: NAD<br>BCS 6/9<br><br><strong>In house diagnostics:</strong><br>None required<br><br><strong>Assessment:</strong> <br>Pruritis on paws<br><br><strong>Treatment: </strong><br>Discussed option of using Zenrelia versus Cytopoint. As Monica has used Zenrelia with good results, elected to continue<br>Zenrelia 8.5mg x 30 tabs. Advised if require long term use then will need LTDM.<br><br><strong>Plan: </strong><br>If need to use zenrelia longer term to book in for LTDM. For now, unsure if this is a once off this season, or start of something longer term OR seasonal at this time of year.<br><br><br></p> </div> </div></html>

Spoke with Monica, Teddy has improved well after his course of Zenrelia, itch stopped after a few days and he occasionally chewed at his feet. No current concerns, advised if his itch returns now he has stopped the tablets then to call and we can work out a plan. Monica happy with Teddys response to the itch tablets (Zenrelia).

History:Presenting for skin issues on the left hind paw again. This is a recurrence of a previous issue in April last year which resolved with a steroid injection and tablets BUT he was PU/PD. Perhaps a non steroidals would work more efficiently for him? The client has been applying neocort intermittently with minimal effect. The licking is now causing panting at night and a soggy foot! The issue is primarily on the left hind paw, but he also licks the left front paw. He is otherwise well, eating and drinking normally.EDDU normallyDiet: Kibble and half a thin roll of chicken, rice, and vegetable mix. Sometimes eats cat food.Preventatives: Currently on monthly parasite prevention. Discussed switching to Bravecto Quantum for cost-effectiveness.Vaccinations: Last vaccination was in August. Due for next vaccination this coming August and will do BQ as well as SR 12 then.Current medications: None.Exam:BAR, happy and friendly but not when his feet are touchedMm pink, CRT <2sEyes NADEars NADental comments if any: Grade 1 tartar, no significant gingivitis but needs and scale and polishSkin / Integument Saliva staining of both left hind and a littel on dorsum of left fore too. Mild secondary dermatitis on the left hind paw from licking. No significant redness or inflammation. Pruritus and secondary dermatitis, left hind paw and a little left fore.- ddx: Allergic dermatitis, habit licking, contact dermatitis.Treatment:Zenrelia 8.5mg 1 tablet SID for 5-10 days (x 10 tabs)Plan:Monitor response to Zenrelia. If no improvement, re-evaluate. Discussed switching to BQ for annual parasite prevention at next visit. Plan to perform annual vaccination, heartworm injection, BQ then Client Communication:Discussed the nature of the skin issue, likely a habit or allergy. Explained the treatment plan with Zenrelia.Advised on the benefits of Bravecto Quantum for parasite prevention. Discussed the plan for annual health check, vaccinations, and dental procedure in August. Provided with a pamphlet to track itch levels.
