No obvious trauma but o concerned as she believes slim is in pain. Otherwise OK, EDDU WNL and no V+D+. Exam: Attitude: BAR Weight/ BCS: 3/5- OK Temp: 37.9 Integumentary: Superficial graze on top of skin fold above nasal planum. Skin not broken, only some minor hair loss. No other laceration/ trauma. Sensitive on palpation Cardiovascular: HR/PR normal, pulse quality normal, rhythm regular, skin tent <1s Respiratory: RR normal, RE normal, auscultation normal Remaining PE unremarkable Problem List/ Assessment: Bruising over wound Medications: Meloxicam given @0.1mg/kg SQ and PO 3 day course Plan: NIL recheck needed. To not exceed 3 day course meloxicam.
