Radiographic findings:
Views (three view thorax)
Assessment (Thoracic radiographs demonstrate mild cardiomegaly with a bronchial / bronchointerstitial pulmonary pattern. No evidence of pulmonary venous congestion or caudodorsal/perihilar alveolar pattern to suggest cardiogenic pulmonary oedema. Findings most consistent with chronic airway disease (chronic bronchitis) with possible concurrent early degenerative mitral valve disease given presence of low-grade murmur.

Plan 
Treatment plan:
Continue prednisolone, bronchodilator therapy, and codeine for management of chronic airway disease and cough suppression. Recommend monitoring resting/sleeping respiratory rate at home to screen for potential progression of cardiac disease. Consider further cardiac assessment (repeat thoracic radiographs, NT-proBNP, or echocardiography) if cough worsens, exercise intolerance develops, or resting respiratory rate increases.
