WEIGHT: 38.00 HISTORY: 05 Mar 2026 05:53:11 pm KAM Presented for acting lethargic at home. Just seems to be sleeping most of the day. Does perk up when goes out e.g. coming here. normal appetite at home Hasn't put on any weight past 8 months (prev weight 38.3). Has been rec weight loss. O has cut back on food. Due for her next heat shortly. Last one 9 months ago and she generally has a heat every 9-10 months hx of: - cutaneous papilloma surgically removed - otitis externa - fish hook ingestion, treated conservatively April 2024 - not UTD vaccintions- prev declined - seen previously for lethargy on walks, rec weight loss prev weight= 38.0. Has been rec weight loss previousl Examination: T:38.0 Capillary refill time 1-2s mm pink and moist HR:128bpm RR: panting demeanor: Bright & Responsive, boisterous eyes/ears: No abnormalities detected oral: DS 1 upper canines - excellent dentition LN: submandibular, prescapular and popliteal WNL cardio/resp: no murmurs, no arrhythmias, normal BV sounds bilat abdo: soft, comfortable, difficult to discern organ detail as overweight BCS: 7/9 Assessment: lethargy DDX overweight vs MSK vs cardiovascular vs endocrine (hypothyroidismn) vs other Plan: Rec TAHP bloods to rule out hypoT and investigate causes of lethargy. Also good idea as 8YO and hasn't had a blood test previously. Will call with results over the next couple of days
