HISTORYPresenting for: Persephone, a 1 year old female cat, presented for frequent urination occurring every 1-2 hours for 1.5 days, producing only small amounts of urine  or nothing, with post-urination genital licking, and a wound on her chin. Defecating regularly.Diet:- Premium dry food (TOTW)- Automatic feeder usedDrinking/Urination:- Going to litter box every 1-2 hours for 1.5 days- Staying in box for 3-4 minutes each time- Producing only very small amounts of urine or nothing- Licks genital area after each attemptCurrent Meds: NonePreventatives:- Advocate spot-on every 2 months for flea/worm prevention- Vaccinations last done April 2025, due for adult booster in approximately 1 monthLifestyle Risk Factors:- Indoor only cat, previously sole cat in the home.- Recently moved into multi-pet household 1 month ago. - Now lives with 2 other cats (5 year old male, 9 year old female) and 1 dog. O felt the cats were all getting along well although noticed new wound ventral chin and showed video footage which was appeared to display intercat aggression. Male cat doesn't like to interact with Persephone. 3 cats roam between 2 levels in the home.- Dog comes and goes, chases cats- Planning to stay in current house for at least 1 yearOther:- 2 automatic litter trays on level 2 in same room, positioned approx. a metre apart. - Following move, changed from tofu and clay litter mix in open tray to clumping litter in automatic trays- Trays cleaned weekly- Has access to fountain and open water bowls- Plays with toys, wants to play with male cat but he doesn't reciprocate- Has scratch post access- Video evidence of fighting with other cats, not happy play interactionsEXAMMentation: BARBCS 5.5/9Behavioural: Very nervous and anxious during examination.Cardiovascular: NADAbdominal: NAD, bladder empty on palpationIntegumentary: Small wound present on chin, possible fight wound.Eyes/ears: nadOral exam: nadUrogenital: Vulva and perineal area appear normalT: NormalASSESSMENT- Feline lower urinary tract disease (FLUTD), first episode. Consider FIC, bacterial cystitis, urolithiasis, bladder wall abnormality, other.Stress from recent house move, introduction to multi-pet household and negative inter-cat interactions likely contributing factors.Facial wound - Possible fight wound ventral chin TREATMENTPain relief - TM Buprenorphine 0.2 mL BID x 5 daysPLANDiagnostics:- Urine collection kit provided for urinalysis to rule out bacterial infection, crystals, and other abnormalities- If unable to collect sample at home or clinical signs persist, return for morning appointment to collect sample directly- If bladder empty on return visit, may admit for day to wait for bladder to fill- Recommend Feliway Friends diffuser for inter-cat anxiety and aggression - purchase starter kit then replace monthly- Recommend Protexin Serenicare for Pers. +/- other catsAdditional Discussion:- Separate cats temporarily to reduce stress and enable slow re-introduction.- Add third standard open litter tray with previous litter type (tofu and clay mix) in separate location for patient. Scoop standard tray immediately when soiled.- Ideal ratio is 4 trays for 3 cat families, positioned in separate areas where cat can see approaching threats- Monitor for ability to pass urine - blockages rare in females but watch for changes- Clean chin wound twice daily with saline solution, monitor for signs of infection- If clinical signs don't resolve or recur, further diagnostics including bloods, urine C&S and bladder imaging/ultrasound will be recommended.- Patient at upper end of ideal weight - monitor portions to prevent further weight gainDiet Recs:- Consider feeding predominantly wet food diet if Persephone will adjust to this.
