Reason: Bloods (Leave Long)History: 8am given gabapentin & trazodone. Had eye ulcer about 2 weeks ago, also on tacrolimus. Bottom of lid. Seen at ophthalmologist. Started on cephalexin and eye has improved. Since starting cephalexin, A week ago started drinking more water +++ Not holding urine any more, now regular accidents in house Decreased appetite - usu a very good eater. Not eating in the morning which is highly unusual. Eating the entire day's food overnight.Last dose of cephalexin was given a week ago. All changes in thirst and appetite and energy started with cephalexin.O has measured 600-700mls of water intake over 24h. Brought first urine in this morning. Meds: On B12, eye drops and iron. Had soft stool initially with cephalexin. No current diarrhoea or vomiting. Examine: QAR, sedated. Mm pink, CRT1s. HR120 no murmur or arrhythmia = PR regular. RRwnl normal effort. T 37.67. Abdominal palpation tense. Peripheral lnns wnl. Weight stable. Laboratory: USG 1.014. Dipstick pH 6. All else negative.Assessment: new PUPD. Isosthenuria on 2 morning urine samples. Plan: vetnostics TAHP. JW to follow up tomorrow (sat). Vital Signs Weight: 12.96; Temperature: 37.67; HeartRate: 120;
