06/03/2026 Presenting Complaint: Recheck for seizure management on phenobarbital

Hx: Genevieve started phenobarbital mid-January following seizures. Last seizure was 23/12/2025. Previously had seizures approximately every couple of months, with two seizures in three to four weeks at one point. Since starting medication, no seizures reported. Side effects noted include increased hunger ("she'll get annoyed when she's hungry and just start tapping at the fridge"), increased thirst, and increased urination. Still anxious, not sleeping as heavily, waking earlier than usual at night. No lethargy noted. No vomiting, no diarrhoea, no excessive coughing or sneezing. Weight gain of 200 grams since last visit. Walks have been reduced recently.

Assessment:
Idiopathic epilepsy on phenobarbital therapy. Tolerating medication well with no significant adverse effects. Side effects of increased appetite, polydipsia and polyuria are expected and manageable. Weight gain of 200 grams noted, not overly concerning but would prefer to see weight reduced by 400-500 grams.

Plan:
Continue phenobarbital half tablet twice daily.
Increase exercise with regular walks.
Blood results to be reviewed Tuesday.
Phone call Tuesday to discuss results and adjust medication dosage if required.
Owner to return for more medication once results confirmed.

Consult Summary:
Genevieve presented for recheck on phenobarbital for seizure management. No seizures since 23/12/2025. Tolerating medication well with expected side effects of increased appetite, thirst and urination. Blood sample collected for phenobarbital level monitoring. Results to be discussed Tuesday with potential dose adjustment.
