25/02/2026 SUBJECTIVE:presented by adam irritated by hindend as if difficulty defaecting, scooting adn licking. holly expressed ags and showed photo of yellow chunky material expressed from both ags. otherwise well, due for vacc. ears ok OBJECTIVE: bcs 3/5 both ear canals clear, no discharge. left ag already empty. difficult to express rt ag, feels as if gas in there, very little contents expressed. ASSESSMENT: ag infection PLAN: last time not sensitive to clav, so start enrofloxacin po 10 dasy rv 1wk to express ags given dhp triennial, pibb sc. disp elocon to use wkly to reduce frequency of otitis flare ups

SUBJECTIVE:presented by mrs irritated by right ear. always a little pruritic. OBJECTIVE: rt ear canal and pinna inflamed, left ear canal bit mucky. cytol- yeast, rt worse than left. skin generally appears ok ASSESSMENT: allergic derm with secodnary otitis PLAN: o opt to try cytopoint, understands may not be that effective for ears. given cytopoint 40mg sc. pmp bid both ears 2 wks then rv (ok to disp more if o requests within 2wks). if ears clear at rv, consider elocon wkly to reduce frequency of flare ups
