-Health concerns: Discussion about specialist referral for left leg ulnar osteotomy surgery (similar to previous right leg surgery that showed improvement)

-Current medications: 3 monthly cartrophen injections

Physical Examination
-Mentation: BAR
-Ears/Eyes/Nose: NAD
-Oral cavity: plaque noted, no gingivitis noted
-MM: pink
-CRT: <2s
-LNs: wnl
-Thoracic auscultation: no heart murmur or adventitious lung sounds noted
-HR: 120
-Abdominal palpation: NAD

Assessment:
1) Healthy to vaccinate
2) Left leg elbow dysplasia requiring specialist consultation for potential arthroscopy and fragment removal +/- ulna osteotomy

Plan:
1) Vaccinated with Pi2 SC + oral BB.
   - Next vaccination due in a year's time

2) 1.1mL Cartrophen injection SC
   - Next due in 3 months' time

3) Specialist referral for left leg
   - Owner to discuss arthroscopy options with specialist
   - Plan to start with arthroscopy and assess need for additional procedures during surgery
   - Request specialist to check suture reaction under right eye while patient is sedated
   - Also to check lump on lower lip at the same time too
