Reason: Weight lossHistory: Zac has been deteriorating and losing weight. Has severe conjunctivitis that is not really responding to the doxy abs.Examination--CANNOT EXAMINE without heavy sedation:Domitor 1ml, butorph 2mls and ketamine 1.5mls IM..GOOD SEDATION as able to examine and take bloods.Examination: Zac has numerous issues; conjunctivitis.WEIGHT loss--abdomen is also distended so concern for liver/splenic dx.PLAN: 1. Bloods taken for full profileLAB--SEVERE LIVER DX, PANCREATITIS, DIABETES MELLITUS and dehydrationZAC WOULD NEED EXTENSIVE intravenous fluids, insulin, monitoring and blood testing.The owners and us as a vet hospital are not able to touch him without him attacking so he would not be able to handle twice daily injections of insulin, blood testing and monitoring.PLAN: 1. O, elected to euthanase as they would not be able to do treatmentPlaced left lateral saphenous catheter and gave 30mls of lethabarb.Passed peacefully.O, would like general cremationsympathy card done (mel)
