05/03/2026 Diagnosis:Gastric small cell T cell lymphoma. Chlorambucil revisit. 8 months post starting Chlorambucil. 

History: Miss Kitty has been doing very well. Leanne reported that Miss Kitty shows no signs of any health issues, and she appears to be joyful and happy. Her appetite and toileting habits remain normal. There are no concerns regarding Miss Kitty at this time.

Physical Examination findings: Bright, alert and responsive.
Mm pink, crt < 2 secs, HR ~152, RR 52, Temp: 38.0 , Wt: 6.23kg
Cardiovascular: no notable murmurs or arrythmias auscultated, femoral pulses good and synchronous
Respiratory: normal respiratory effort, normal bronchovesicular sounds on auscultation.
Abdomen: soft and comfortable, no masses or organomegaly 
Integ:no lesions, wounds and ectoparasites noted
Lymph nodes: all peripheral lymph node palate normally 

Laboratory results: Standard profile pending 

Treatment: 200mg Gabapentin administered in hospital 
 - Chlorambucil 2mg (Leukeran) x 25.00
Give 1 tablet Monday, Wednesday and Friday. 
CYTOTOXIC – Wear gloves and wash hands afterwards, do not break or split tablets. Keep refrigerated. 
 - Prednisolone 5mg Tablets (Pred-X) x 30.00
Give one tablet every other day. May cause increased appetite, thirst, lethargy.
 - Cobalamin (Vitamin B12) (Cobalazorb) Capsules x 20.00
Give 1/2 capsule once a day.
 
Comments: It was lovely to see Miss Kitty today.We will see Miss Kitty again in 10 weeks time for physical assessment, CBC and imaging (abdominal Ultrasound).  Please feel free to call or email if you have any further queries regarding Miss Kitty.
