ASSESSMENT:Kiba presented for a recheck post-blood transfusion. In-house diagnostics show an improved PCV of 30% compared to 26% on 03/03/2026. The patient is clinically stable on current immunosuppressive medication. TREATMENT:  Medications dispensed:- Gabapentin full box dispensed.- Trazodone script provided for sedation for future visits. PLAN:Continue current medications (Prednisolone, Cyclosporine, Clopidogrel) at current dosages.Contact ARH/ZY to confirm the long-term medication plan, including the protocol for tapering Prednisolone (likely to commence in two weeks).Revisit scheduled for next Tuesday for re-evaluation.Home into owner's care. CLIENT COMMUNICATION:Discussed the improvement in blood results. Advised to continue all medications as prescribed.Advised on signs to monitor for, including weakness, collapse, or increased lethargy.
