HISTORY
Presenting for: Patient presented for defecation difficulties and anal gland issues.
Current Meds: Glandex for anal gland issues

EXAM
Rectal: Both anal glands small in size with thick yellowish brown material expressed. Right anal gland slightly difficult to express. Faeces very hard suggesting constipation. Anal region erythematous from irritation. Patient attempting to defecate multiple times without success.

ASSESSMENT
Constipation - evidenced by hard faeces and difficulty defecating
Anal gland impaction - bilateral, with thick secretions and associated perianal irritation

PLAN
New Meds/Tx:
- ActiLax 3 ml daily at night as laxative for constipation

History
Presented for routine check-up and anal gland evaluation.
Eating, drinking, defecation, urination normal.
No vomiting or diarrhoea.
Vaccinations up to date.
Diet consists of single-protein feeding: lamb in the morning and crocodile at night.
