Reason: Straining To UrinateHistory: Milo is approximately a six-year-old male neutered cat who has been brought in this evening with concerns over his urination. He is an inside only cat who lives with his littermate and owner's report no major issues between the cats, although they do play fight at times. He had an episode of FLUTD approximately one year ago. He did not obstruct at that time.He has been on urinary food, Royal Canin, dry and wet, and over the last few months has had standard wet food added in and also some dry food added in as well. Today he has been noted to be in his litter tray and seemed to be vocalising and uncomfortable and was leaking urine.Examination Findings:  A little anxious in hospital, but vital signs fairly normal, did not take temperature. Bladder is approximately 5 cm. When pressure is applied, bladder becomes very tense and cat spurts out small bits of urine. No crystalline deposit noted on the prepuce. Assessment: At this stage does not appear to be obstructed, appears to be FLUTD. Differentials include idiopathic, possibly most likely. UTI unlikely, bladder stones possible. Underlying stresses may be in play that owners are not able to appreciate at this time. Plan: Given this is an acute onset with a previous FLUTD episode, at this stage treat as idiopathic first offence episode. If ongoing recurrent or worsening will require further workup including imaging of the bladder, blood work and urine testing.Discussed with owners, they agree with approach and discuss moving forward, staying on urinary only food and for the next two weeks, wet urinary food only if possible. Discussed risks of blocking and mechanism as to why that's an issue. Owner aware of the signs to watch out for.Treatment: Buprenorphine 0.3 mL IM at discharge. Sent home with supply for TID Q8-12 hours.Cystophan, owner's used before and felt helped, one capsule twice a day. Add extra water into the urinary food and close observation for risk of blockage.
