Pre Anaesthetic Examination: HR 80        CRT 1-2sec         MM pink, moist          RR panting
General Anaesthetic:
 - Premed - acp/meth IM  @9.16am
 - Induction - Alfaxan IV @ 9.43am

Procedure: EUA
Lt canal clear with a small amount of brown waxy material near TM. TM intact, no FB present.
Rt ear clear, no dishcarge. GS at the end of horizontal canal near TM. - Removed. TM intact. Small amount of bleeding from mucosa after GS removal.

Good recovery.

Treatment: Dermotic bid Rt ear for 5 days to control inflammation and prevent infection formation.

Acute onset head shaking and scratching at L ear since this morning 
Owner attempted to look into the ear with a torch and did not see anything.
Dog lives on nearly 2 acres with grass seeds present on the property.
Breakfast given at 07:00 today (about 1/2 a cup of kibble).
Drank water around 07:45-07:50 today.
No vomiting or diarrhoea within the last 48 hours.
Owner suspects a grass seed may be involved.
Insurance is in place.

On any medication: Meloxicam PO SID, Gabapentin 300mg PO BID, Clomipramine 25mg PO BID. Given ~7-7:30am

Haematology: Not indicated today; recent blood work performed a month ago per owner.

Ear cytology: Indicated if no FB found in either ear

Assessment:
Differentials: Possible aural foreign body (e.g., grass awn). Otitis externa is also a consideration given pruritus and head shaking. Definitive assessment pending otoscopic examination under sedation or anaesthesia.

Plan:
- Admission for sedated otoscopic examination - remove any FB if present 
- Ear cytology if no FB present

History:
Owner reports one episode of redirected aggression last week when a sheet was shaken in bed after the owner reacted to a mosquito; Bordeaux immediately attacked household dog Fuji.
Owner describes these incidents as stress or fear-related discharges of emotion rather than intent to harm; frequency low with last prior episode approximately 2 months ago.
When overwhelmed, owner notes Bordeaux’s eyes appear dark; signs typically occur in the evening at end of day.
Dogs are routinely separated at times at home and will be separated when owners are away; Fuji stays in the dog room with the back door boarded to prevent chewing.
Previous house sitter arrangement did not go well; new sitter is in training as a behaviour trainer and trusted for special needs care. Owners will also utilize neighbours on 2 weekends during current trip.
Owners have implemented a diet to reduce weight; initial first week weight loss noted, then plateau.
Recent radiographs focused on skeleton; chest and full abdomen were not comprehensively imaged.

Changes in behaviour: Occasional redirected aggression toward household dog triggered by sudden stimuli and end-of-day stress; otherwise described as a sweet temperament between episodes.
Changes in eating/drinking: Owner reports diet restriction to one cup of food per day; treats have been reduced after a period of increased treats during snake training.

Current medication regime:
1. Metacam SID
2. Gabapentin 300mg BID
3. Clomipramine 25mg BID
4  weaned off clonidine

Clonidine will be used if behaviour flare while family away

Pain Score (/10) - No obvious pain events noted

Assessment:
Stress- and fear-related redirected aggression toward household dog, triggered by sudden environmental stimuli and cumulative daily stress. Chronic pain may contribute given ongoing analgesic therapy and weight management focus.
