05/03/2026 HISTORY - Dog has been scratching paws and ears, causing irritation and hair loss. - Previous treatments include Cytopoint injection, Claratyne, and various topical applications. - Dog has a history of allergies. - Previously attempted to train as an assistance dog but discontinued due to difficulty managing allergies. - Concerns about the dog's behaviour potentially being a self-soothing response due to stress. EXAM Paws: Irritated from scratching Ears: Scratching noted, owner unable to examine closely due to dog's discomfort DIAGNOSIS - Allergic dermatitis (suspected based on symptoms and history) PLAN - Continue with Claratyne, switch to adult dosage as per owner's plan to purchase. - Apply topical treatments as previously prescribed. - Owner to manage environmental factors and grooming to reduce irritation. - Recheck and monitor response to Claratyne and topical treatments. - Discuss further diagnostic tests or adjustments in treatment based on re-evaluation. osurnia in 1 wk | 05/01/2026 HISTORY - Lethargy, decreased appetite, not excited about food - Shaking head frequently, rubbing against walls - Left ear treated with BID drops, improvement noted, no discharge visible - Right ear started showing symptoms a week after left, treated at SAH - No vomiting, normal drinking and urinating habits - Normal activity levels at dog park, lethargic at home - Normal body temperature, good colour, lymph nodes normal - Flea and tick prevention up to date EXAM Ears: Left ear shows no signs of infection or discharge, dislikes otic examination so limited visibility right ear treated but still has s=a few yeasts present issues General: No fever t-38.5 mm pink Eyes NDA , good overall lymph nodes normal Chest clear Hr-88 no murmur NDA abdo palpation coat and skin NDA DIAGNOSIS - Otitis externa in both ears L under control more recent in right ear and ongoing PLAN 1. Continue treatment for right ear with prescribed medication- Osurina 2. Administer second dose of Ozonia in right ear 3. Perform repeat ear smear in 2-3 weeks to monitor progress 4. Annual checkup to be scheduled after current issues are resolved
