HISTORY: Peter presented with Mayo. EDUF all ok. No v+/d+; no coughing. Ran out of apoquel and pred. Starting to get a couple of scabs pop up. O doesn't think apoquel helps at all and is actually wanting to drop this and try with just pred alone.

Current medications:
-Apoquel 8mg PO SID
-Pred 5mg SIW-EOD
-Malaseb baths
-Elocon for ears SIW

Diet: no success with hypoallergenic or diet trials.
 
CLINICAL EXAM: 
Demeanour:  BAR
BCS:  6/9  Weight assessment: Good body condition.
HR: No heart murmur detected
RR: Chest sounds clear
MM pink, crt<2secs.
Abdo palpation: NAD
LN: NAD 
Eyes: NAD 
Ears: NAD 
Dental grade: 2/4 (rec s&p)

Skin: generalised alopecia all along trunk / ear pinnae / tail base. 2x crusty scabs in L axillary area, rest of skin okay at the moment.

Other findings: lumps/lesions as per April 2024 hx. L proximal vulva lump is currently v small in size (o reports this got bigger at one stage and is small again) - will be quite hard to get conclusive FNA with size (4-5mm), but offered this anyway as want to rule in/out MCT with this hx - o declined today but will consider if it gets bigger.

TREATMENT:
Vaccination: Duramune C4 SC + BS oral (tolerated well)
Parasite control: UTD
Anal glands: expressed, mod full, normal consistency

For skin: O understands about pred side-effects longer term but has been managing to keep this dose relatively low. Discussed option of trying zenrelia in future instead, will think about this.
Last bloods for ongoing meds were Aug 2023 - rec repeat, o declined and will do this next time.

Dispensed neocort to use on the scabs noted today, and now with pred again hopefully this will settle. Okay to dispense 2w course of ceph if the area worsens and o calls back <2w.
