Reason: Swollen AnusVerified: NoHMM - 25/02/2026History:- 7-year-old Cocker Spaniel named Quinn- Presenting for anal gland issues - straining, scooting, and licking for 5-6 days- Large lump noted at top of anus- Previous lip fold removal and dental cleaning by Sasha- Chronic eye discharge with crusty material, clear to stringy discharge- Long eyelashes that poke into eyes, owner trims between grooming- Currently on Royal Canin Cocker Spaniel food with wet food sachet- Severe separation anxiety since death of companion dog Murphy 4 years ago- On Fluoxetine 10mg daily for 4 years (originally 20mg but reduced due to tremors)- Cannot be left alone, requires pet sitter when owners leave- Previous Cocker Spaniel Murphy had anal gland problems around age 7Examination:Eyes: Clear discharge, no conjunctivitis, no significant irritationTeeth: Good conditionAnal glands: Very full but no signs of infection or abscessationDIAGNOSIS- Impacted anal glands- Chronic epiphora (eye discharge) likely secondary to hair irritation or blocked tear ducts- Separation anxiety (ongoing management)Plan:- Anal glands expressed today- Recheck anal glands in 1 month with nurse or during behaviour consultation- Diet change to Hills Science Diet for dental health- Increase dietary fiber with cooked pumpkin, raw carrots, or cooked vegetables- Continue Fluoxetine 10mg daily, prescription to be written- Referral to Paul for behaviour consultation regarding anxiety management- Monitor eye discharge, continue trimming eyelashes as needed- No antibiotics or anti-inflammatories required for anal glands    Vital Signs    Weight: 18.6;
