Appointment Notes: D+ & V+ still eating In with AlisonHistory: On Saturday got a message from pet sitters to say he was having some D+ Gave boiled rice and chicken O's picked him up on SundayThought he was a bit off when O came to picked him nightHad some vomiting the following, small patches here and there Yesterday vomited too, but no vomit today No further D+ since Energy has been normalYesterday ate normally, eating treats fine Not a scavenger Objective: BAR, grumbling initially but good for examination Integument: no obvious skin tentingEyes/ears/nose - nadOral: MMs P+M, CRT <2secCardio: HR140bpm, grade I-II LHS systolic heart murmur, femoral pulses strong/synchronous Resp: RR wnl, lung fields clearGIT: mild discomfort on mid-caudal abdo palp, otherwise soft Rectal: normothermic, scant semi-formed faeces in rectum Assessment: Vomiting, diarrhoea - ddx: primary GI (gastroenteritis, dietary indiscretion, FB, others) vs secondary GI (pancreatitis, liver dx, kidney dx, others)Options discussed:1/ Symptomatic treatment - bland diet, prokolin +/- anti-nausea2/ Further investigations - bloods +/- imagingO opted for option 1. Treatment:ProKolin 30mlPlan: RV in 24-48hrs if no improvement for further investigations (bloods +/- imaging) Discussed heart murmur.
