<p>ASSESSMENT<br>Back pain - DDx: intervertebral disc disease, muscle strain, soft tissue injury</p>
<p>PLAN<br>New Meds/Tx:<br>- Injectable meloxicam 0.32ml s/c<br>- Oral meloxicam sid 7 days starting tomorrow night<br>- 100mg gaba bid starting this am</p>
<p>Additional Discussion:<br>- Expect improvement by tomorrow, may return to normal<br>- Continue medication for full week even if symptoms resolve<br>- Treat normally and encourage movement - current theory suggests activity is beneficial for back pain rather than rest<br>- Monitor for development of wobbliness in hind legs which would indicate more serious condition<br>- Recheck if not improving</p>
<p>Client Comms:<br>- Return for recheck if not improving by tomorrow. May require imaging. </p></html>

HISTORYPresenting for: Archie presented for evaluation of decreased appetite and slower eating habits.Diet: - Decreased appetite noted over the past week - Takes longer to consume meals - Does not finish breakfast consistently - Owner reports he is less motivated to eat and will leave food for extended periodsNot able to exercise like normally does.Panting alot.Drinking more than normal. Yelping intermittently when touched in different areas. EXAMMentation: bright and well in consult. Wt: 7.9BCS:nT: 38.3HR:112 normal. No murmur or arrhythmia. R: PantingMM: pink. Dental disease- moderate with tartar and periodontal disease. Hydration: no skin tentingBehavioural: normalEyes: normalEars: normalNose: nThroat:nOral: see aboveCardiovascular: heart and lungs normal. Respiratory: Lungs clear. Abdominal: tense- not able to palpate well. Lymph Nodes: all LNs normal. Integumentary:nNeurologic:nMusculoskeletal: normal spine, FLs and HLs. Yelped once when neck turned to LHS but this not repeatable. Urogenital:nRectal:nASSESSMENTDecreased appetite - slower eating pattern over past weekDrinking more than normal. Panting. Less able to exercise/. PLANFull bloods. MM- bloods normal. O will monitor for the next few weeks. Next step sedate, xray chest and abdominal ultrasound. May also need urine test. O will let us know and get back to us if wanting further testing done.
