05/03/2026 Diagnosis: Large cell lymphoma, CD21+, MHC II + (flow cytometry and cytological diagnosis). CMOP Week 8 Mitoxantrone


History:

Physical Examination findings: Bright, alert and responsive.
Oral:moderate periodontal disease
Heart & lungs  left-sided heart murmur grade IV/VI, no arrythmia noted on auscultation. Normal respiratory rate and effort, normal bronchovesicular sounds on auscultation.
Gastrointestinal: soft comfortable abdomen on palpation.
Rectal exam: soft stools on glove, no lymphadenopathy
Integument: no obvious lesions or wounds 
Otic/Ocular: normal ocular and otic structures
Lymph Nodes: no peripheral lymphadenopathy noted
Mm pink, crt < 2 secs, HR 148 , RR pant , Temp 38.8, Wt: 38.2

Laboratory results: CBC performed in house with manual blood film assessment.
Hct: 0.454
WCC:7.42
Neuts: 4.27
Platelets: 382

Manual differential count:  Neutrophils 69%, Lymphocytes 13%, Monocytes 2%, Eosinophils 16%.
Platelets: clumped and adequate
Morphology: Normal 

In-house biochemistry: Urea 10.6 2.5 - 9.6 mmol/L and ALP 280 23 - 212 U/L. All remaining parameters were within normal limits 

Treatment: xmg mitoxantrone (5.5mg/m2) was administered IV via a 22g catheter in the x vein using the closed injection Equashield system.
Use anti-nausea (maropitant/cerenia) as needed for nausea, inappetence or vomiting
