Straining to urinate since yesterday.Owner noticed straining since yesterday after a recent change to clumping litter. Hope did a large urination initially, then began straining. Owner started leftover amoxyclav yesterday and this morning, however straining persists. Small amount of blood seen in litter today  Previous episode of cystitis with significant haematuria noted historically.350g weight gain since 3 months ago EDD normal, no V+D+ Pre-GA bloodwork last performed 4 months ago, renal function WNL Examination:Temperament/Demeanor: BARChest Auscultation: No murmur/arrhythmias, lung sounds clear. Synchronous femoral pulsesOral: Gums pink and moist, grade 2-3 dental disease Eyes/ears/nose/throat: Nad Abdo palp: Comfortable and soft. Urinary bladder small and difficult to palpate as overweightSkin/coat: Nad Gait: Nad MSK: Not assessed LNs: Nad Neuro: Normal mentation Rectal/urogenital: external genitalia normal Laboratory:Attempted US-guided cystocentesis - urinary bladder almost empty with thick wall, unable to collect urine. Towel in carrier smelled like urine so Hope likely urinated and emptied bladder Assessment:Acute stranguria and haematuria - FLUTD (idiopathic cystitis vs UTI vs crystalluria/urolithiasis)OverweightGrade 2-3 dental disease Plan: Recheck and retry cystocentesis in 2-3 days (not appointments available tomorrow) Start meloxicam 0.05mg/kg PO SID for 4 days for anti-inflammatory pain reliefGabapentin 8-15mg/kg PO BID for 3-5 days for anxiolysis as suspected FICO has already returned to previous litter type. Consider Hills Urinary Metabolic diet for weight loss. Target weight 5kg.
