Appointment Reason: SickSubjective:- Presenting for vomiting and bloody diarrhoea with mucus. The owner reported that Sachi was listless this morning and vomited in the waiting room prior to the consultation. A photo of the stool was shown, demonstrating mucousy consistency with blood present.- This is the first episode of bloody diarrhoea that the owner is aware of. The owner had visitors overnight and the dogs were outside during this time. There was no stool in their bed overnight, however some was noted in the kennel. Sachi ate last night and had been given a dental stick. The owner mentioned Sachi was being clingy and sooky this morning, and when a visitor named Carly came around, Sachi went straight inside to her rather than exhibiting usual behaviour.- The owner expressed concern as another dog in the household previously had similar symptoms at approximately thirteen years of age and required surgical intervention for internal issues.- Dietary history includes homemade turkey mince and beef mince with vegetables and grain, along with kibble in the morning given in small quantities, approximately half a handful. Dental sticks were recently introduced. Occasional treats including carrot are given.Objective:- Weight: 3.4 kg- On abdominal palpation, Sachi was noted to be tender.- Some fur was trimmed from the perianal area as it was soiled and not manageable.Assessment & Plan:1. Haemorrhagic Gastroenteritis- Assessment is haemorrhagic gastroenteritis, likely related to dietary indiscretion or ingestion of something that has disagreed with the gastrointestinal system. Given Sachi is a young dog, this is considered a self-limiting condition that should respond to symptomatic treatment. The owner was reassured that this is unlikely to be similar to the previous dog's condition which presented at an older age.- Symptomatic treatment initiated. An injection of Serenia (anti-nausea medication) was administered by Mark during the consultation. Sachi was noted to be brave during the injection, though there may be some transient discomfort at the injection site.- Instructions provided to withhold food and water for a couple of hours following the injection.- Medications dispensed: Gluprocolon, a probiotic paste, to be given at 2 mL twice daily. This can be administered directly into the mouth or placed on top of food. Metronidazole, a quarter tablet to be given morning and night. Maropitant, half a tablet to be started Friday morning for continuation of anti-nausea therapy, with four doses provided to cover the weekend if required.- Owner advised that improvement may take a couple of days and there may still be some blood and mucus in the stool tomorrow, but resolution is expected fairly quickly.- If symptoms do not improve, further investigation including blood work and imaging may be considered, however this was not indicated at present given the likely self-limiting nature of the condition.- Owner is going away for the weekend and has arranged for someone to watch Sachi who will be able to administer the medications.
