HISTORY:ok yesterday but overnight restless and going out a lot. this morning seemed to have trouble passing stools. 
EDFU straining to pass stools. x2 this morning and did not pass anything. 
no vomiting. 
had some treats this morning. not offered food other treats this morning ( doesn't normally get food in the morning) 
-Diet leaps and bounds barf patties, advance dental and some sardines and egg alternating each night. 
-Env indoor outdoor - dog door

SUBJECTIVE: BAR
OBJECTIVE: T= 38.1 MM  pink CRT <2 sec HR 112 = FP RR panting RESP:NAD
BEHAVIOUR ASSESSMENT:Happy dog. NB owner warns that he is not a fan of people coming at him quickly. owner manages the dog well but not a fan of rear end - temp and rectal, hiindlimbs  - handled himself very well. no issues. 
BCS: 4/9
GIT:increased gut sounds, oral exam grade 2 dental disease. 
U/G:NAD
NEURO: NAD
M/S: no pain in hips, hindlimbs or spine 
SKIN: NAD
OPHTH:NAD
OTIC: NAD
LYMPH/N:
ASSESSMENT:straining to pass stool?, restless
Tx OPTIONS:
TREATMENT:synbiotic, bland diet. 
PLAN:bland diet until normal stools  .  recheck if no improvement.

DISCHARGE
Mexico has been straining to pass poo today. He may be experiencing some large bowel diarrhoea where he  may strain and pass minimal faeces. He does not have any faeces in his rectum at this stage so we will support him with some probiotics and bland food ( Hills Gastroenteritis prescription diet or boiled chicken breast and rice) . This should be fed until at least 12-24 hours after his first normal stool and then slowly back to his normal diet. 
If he does not improve he should be rechecked.
