S: Uncomfortable and not sleeping. Very itchy with his ears. EDUF otherwise. No previous issues with skin or ears. Not specific with ears. No current medications. Preventatives u to date. O: not able to assess ears effectively. Smear unremarkaable. Inflammation in right ear but not that much discharge. A: GS vs allergic vs other P: Steroids given suspect general allergy and otoflush. Discussed sedated examination. Discussed derm referral (DE).

HISTORY: Presenting for sore tail BIOP for 3 months, rescue dog Went swimming yesterday around 15 mins at the beach Today sore in tail, painful, bares teeth when touched. EDDU normal, no CVDS Pre-existing conditions/ current medications: NONE EXAMINATION: BAR, BCS 6/9, T not taken Eyes: nad Ears: nad Oral: nad MM: pink moist crt 1-2s HR: 120, no HM or A. PUlse strong and synchronous RR: panting, no crackles or wheezes Abdomen: tense, non painful LN: nad MSK: painful on tail palpation. Can make in full range of motion ASSESSMENT: Painful tail. ddx. swimmers tail > fx PLAN: Loxicom 0.2mg/kg SC Loxicom 0.1mg/kg PO SID for 4-6 days start tomorrow Neurontin 15mg/kg PO TID COMMUNICATION: rest at home, swimmers tail and recur
