Treated w prevomax & lectate. Represented this morning (Wednesday) as still not interested in food, no further vomiting, still bit lethargic. Admitted for the day for IVFT, bloods, abdo u/s & UA. See attached history, summary:Mild lymphopenia, low normal K 3.5 (3.5-5.8), rest wnl Abdo u/s had few non-specific findings (hypoechoic area of liver, hypoechoic areas of L kidney), sediment in bladder Bacteria on UA, culture sent, commenced empiric clavHad prevomax 5pm, buprenorphine @12pmExamination: BARBCS 6/7MM pink/moist, CRT 1.5s, skin tent normalHR 200, pulses strong/synchronous,Resp rate 32, normal effort Abdo palp soft & comfortable LNs nadMSK nad Temp 38.3BP 153/102 (115), 152/85 (105)Laboratory: Epoc PH 7.35HCO3 25.1 HBE -0.5 H Ca++ 1.36 HAssessment: LethargyinappetenceVomiting? No vomiting or regurgitation. Client Communication:ST Nick at 640am - advised settled and comfortable overnight. Eating small amounts - initially only hand fed but then once we realised he didnt like the bowls eating off of a licky matt! Advised would still recommend another 12hrs of care - Nick confirmed had made arrangements with RV to get back there for 9am.
