<html><meta http-equiv="Content-Type" content="text/html; charset=utf-8"><div> <p>Hx: (what and how long)<br>3-4 week duration (since prior visit)<br>Improved while on medication, lesions over neck improved in redness, no further discharge, fur not coming out in clumps there.<br>Gradually noted crusting and clumping in small patches over the rest of the body; skin in between dry and flaky, shedding fur in large volume including tufts.<br>Had seemed okay while on pred and antibiotics.<br>Changed diet to single protein (kangaroo) around the same time as last visit. Has had kangaroo food before, but not as sole food. No similar hair changes.<br>Mild itch in one spot only, where now lots of discharge.<br><br>Clinical exam: (tests and exam)<br>Scattered regions of minimally exudative crusting and generalised skin flaking, shedding fur easily with patting and some regions, tufts coming out of skin.<br><br>Deep skin scraping; impression smear; hair pluck - superficial infectious dermatitis, telogen phase hairs?<br>Blood profile, T4 and cortisol level testing in clinic - NSF<br>Send samples (allergen testing - environmental and food based)<br><br>Any current conditions being treated:<br>May be a continuation of prior issue vs recurring similar issue<br><br>Diagnosis and treatment:<br>Bacterial superficial dermatitis (scattered, extensive) + exudative (local)<br>Dermis changes<br><br>Follow up required:<br>Yes - update over phone +/- revisit pending status</p> </div></html>
