04/03/2026 REASON- Follow up ears HISTORY- Rescheduled from last week - last day or two been head shaking again but was fine before this. Today had a v+ and some D+ may have eaten something, ate and kept it down this afternoon. Diet: lyka, not a fan like she used to be, allergic to chicken Medications: none EXAM- Demeanour: BAR BCS 3/5 HR: 128bpm RR: pant T: 38.3 CRT: 1sec MM: pink & moist CHEST: NAD EYES: NAD EARS: clean, L ear pink, minimal discharge - CYTOLOGY: 1-4 yeasts per HPF both ears and scant neutrophils, better than they have been prev. NOSE: NAD ORAL EXAM: 0/4 SKIN: NAD ABDOMINAL PALPATION: NAD, non painful, slightly gurgly MUSCULOSKELETAL SYSTEM: NE normal gait ASSESSMENT: Improvement with meds, ok to go onto elocon lotion Tx: Ears cleaned otoflush+ 1ml elocon once a week both ears - script on file for long term use PLAN: Recheck INB otherwise every 6mths for script update Recheck sooner if ongoing V+ or D+
