Reason: Conjunctivitis?Appointment Notes: tmHISTORY:  Presented for assessment of right eye discharge. Owner picked Mika up from a boarding kennel today after a 2-week stay and noticed pus-like discharge from the right eye approximately 2 hours prior to presentation ("I thought it was just like a bit of sleep around her eyes... it looked a lot worse"). Owner wiped the eye with a wet tissue but left most of the matter. Mika is otherwise bright and happy. Has been seen blinking frequently and trying to rub her head. There is some redness noted in the eye. She is holding the eye open. History of a foot pad infection a couple of years ago. She has lost weight during her stay at the kennels, from ~6.5kg down to 5.3kg. Current meds: noneVaccination: UTDParasite Prevention: UTD Pre-existing conditions: noneAccess to toxins eg. Rodenticide: none EXAMINATION:Gastrointestinal:- Oral: normal, no mouth pain, dental score (1-4): 2-3- Abdominal: no detectable pain on abdominal palpation- Rectal: not performedUrinary: NSFReproductive: NSFIntegument/Ears: NSFLymph Nodes: NSFEyes: Right eye has marked conjunctival hyperaemia and moderate mucopurulent discharge. Left eye NSF. Fluorescein stain applied to the right eye was negative for corneal ulceration.Demeanour/Behaviour: not discussed PROBLEM LIST:- Unilateral (right) mucopurulent ocular discharge- Unilateral (right) conjunctivitis- Weight loss DIFFERENTIAL DIAGNOSIS/ DIAGNOSIS:Infectious conjunctivitis DIAGNOSTICS:Fluorescein staining of the right eye was performed and was negative for corneal uptake. ASSESSMENT: Mika presents with a history of right-sided ocular discharge and conjunctivitis, noted after being collected from a boarding kennel. Examination confirms marked conjunctivitis without evidence of a corneal ulcer. The presentation is consistent with infectious conjunctivitis, likely acquired at the kennel. Significant weight loss was also noted. TREATMENT:  - Antibiotic eye ointment: Apply a strip the size of a grain of rice into the right eye BID for 7/7.- Metacam oral suspension: Dose for 5.3kg SID with food for 5/7. First dose administered in consult. PLAN:Continue medications as prescribed. Monitor for improvement. The other dog in the house should be monitored for similar signs. Re-evaluation if not improving within 5-7 days or if signs worsen.Home into owner's care. CLIENT COMMUNICATION:Discussed diagnosis of conjunctivitis, likely contagious from kennel stay. Advised on application of eye ointment and oral Metacam, stressing Metacam must be given with food. Advised to watch the other dog at home for signs of conjunctivitis. Red flag advice given: return immediately if Mika starts holding the eye shut, tearing increases, or there is any change to the colour/clarity of the eye (e.g., becomes hazy or blue). Advised that most cases resolve well with treatment, but a recheck is recommended if signs persist after 5-7/7. Addressed weight loss and advised to monitor appetite.
