04/03/2026 Anaesthetic - Pre-anaesthetic Findings(TPR) - TPR unremarkable. Has been seen by NA for a firm lump 4-5 cm diam sl R side ventral neck/thoracic inlet region. FNA has not been performed as too diff to restrain as hyperA Premed - medet/meth . Induction Agent(s) -val/ket . Catheter location - L Ceph . Fluids Y . Maintenance Agent - Isoflurane HR Comment - adeq Po2 Comment - adeq ETCO2 Comment -na BP Comment -adeq Temp Comment -adeq Other Anaesthetic Drugs -nil Recovery - Unremarkable. Surgery Record - Procedure - lumpectomy . Positioning -dorsal Clipped, Vacuumed, Prepped Chlorhex/Alcohol. Incision Site(s) - ventral neck etc Findings -firm sl friable poorly encapsulated mass surrounded by fat and under the first layer of muscle. Procedure/materials - blunt dissected free, some of the tumour broke away so was resected separately. Then went back in and removed a lot of the surrounding fat so the sample sent for histo won't have a margin anyway so interpret as such. Discussion -nails trimmed and cauteried Closure/materials -subcut contin pdo tacked down to deeper layer. skin rivermed crucifix Outcome - adeq Medications -meloxicam, ceph Client Communication - went as well as able and will send of for histo and likely long term monitor the general region etc. NB. Standard post op care includes 1/ Keep wound clean and dry and no bath for 1 week, 2/ Small meal only tonight, and 3/ Make sure not licking/pulling at wound otherwise will need an eliz collar. Recheck 2 weeks
