Karen presented Minta today as she is inappentent again. 
Lost her appetite about 10 days ago. Is still eating supermarket roast chicken. For the first couple of days did eat the chicken mixed with her dry food. Still eating the dental stick. 

Last saw the dog 3.5 weeks ago for vaccinations. 
Owner says she has spoken to someone here in the last week about the loss of appetite. 

Normal diet: RC kibble (mini schnauzer) + dental sticks
Haven't tried feeding her a different type of RC dog food. 
Wouldn't eat Optimum dry or wet food.
Has always been a picky eater. 
No bones or raw food in the diet. 

No vomiting or diarrhoea that owner can think of. 
About 3 weeks ago she actually vomited but none since then.

No current medications. 
No other concerns with how she is going. 

Examination:
Minta was relatively BAR on presentation. 

Body weight: 8.2kg (3.5 weeks ago was 8.4kg, so lost 200g which is nothing significant) 

HR= 96bpm, regular-regular rhythm, no obvious murmur audible. Strong and synchronous femoral pulses.    
RR= 18bpm, clear lung fields with normal respiratory sounds and effort. No crackles or wheezes.     
MM= pink-pigment, moist. CRT= 1-2sec     
Temp= 38.5oC 

Soft and comfortable on abdominal palpation. 
No enlargement of peripheral lymph nodes.

Assessment: 
Very fussy dog - is still eating 

Treatment: 
Dispensed 7.5mg Mirtazapine PO as required, can only be given daily 

Plan: 
Revisit if no improvement 
Trial a different RC food

History
Had C5 last year (duramune) but reminders say she needs this now?  Due for KC + HW
8.4 kg note weight loss.  Owner says she is well aside from a picky appetite.
EDDU (but sometimes fussy with food)
No vomiting/diarrhoea
Energy has been good, happy for long walks

Examine: BAR
heart and lungs normal rate and rhythm
Skin - many soft lumps, some sebacous cysts, one has a bit of a crust on it but looks fine
MSK - a bit stiff, but good for age.  
Oral - MM pink moist CRT <2, dental grade 0-1

Assessment:
Fit to vacc

Plan:
KC vacc today + proheart
Owner requests cartrophen, discussed usual dosing, but owner wants to do less frequently - may come back in 3 months.
Owner requests appetite stimulant - I suggested diet modification first.  Fussy dog!  May be difficult.  Can revisit this.
