Appointment Notes: Puncture in legHISTORY:Seemed a bit off the past few days. Noticed blood on the towel that he has been laying on. Then noticed his stifle was very swollen and an open wound on the back right thigh area. Unsure of how it happened. Drinking a lot of water. Ate chicken today.Gave NexGard Spectra a couple days ago and he has been off food since. Happens every time they give the medication  but comes good after a few days. EXAMINATION:Subjective: BARTemperature: 5:45pm - 41.0, 8:15pm - 39.8. Thoracic Auscultation: Lung and heart sounds NADMusculoskeletal: Large amount of Oedema in the right stifle. Integument: NADDental: C4G1. LNs: Normal size NADAbdominal palpitation: Soft and comfortable. Ears/Nose/Eyes: Clean. Skin: Right inner thigh has a 7cm x 4cm discharging wound. ASSESSMENT:Fever. Cytology - Neutrophils and Intercellular cocci. Pre-surgical: Creatine mildly elevatedCbc: Wbc, Neut, Hbc, Hct - elevatedTREATMENT:Meloxicam 1.5ml administered scMetrobactin 500mg tabs - 2 tabs PO Amoxyclav 500mg tabs - 1 tab POPLAN:Hospitalization overnight.Wound flush and debride under anaesthetic tomorrow. May need Penrose drain placed. 34/03/2026SURGERY:Closed with 3/0 PDO intradermal sutures and 2/0 Supramid external sutures.  TAKE HOME MEDS:Meloxicam oral suspension - 2.5ml SID for 7 days. Amoxyclav 500mg - 1 tab BID for 7 days. Metrobactin 500mg tabs - 1.5 tabs BID for 7 days. INSTRUCTIONS:Recheck in 5 days to remove Penrose drain. Recheck in 10-14 days to remove sutures. Exercise restriction for 14 days. E-collar recommended for 10-14 days. TK
