18/02/2026 HISTORY: Ruby has been experiencing gritty ears for approximately 1.5 weeks, which have been cleaned daily by the owner using wipes, no head shaking or scratching observed. No changes in appetite or behaviour noted. No paw licking or rashes observed. Was at the groomer this morning who noticed that ears had discharge CLINICAL EXAM: Demeanor: BAR, nervous but nice Ears: thickened crusty pinna Eyes: NAD - clear Nose: NAD - no discharge or changes Oral cavity: mm- pink & moist, no abnormalities noted Lymph Nodes: NAD Cardio-respiratory: heart sounds regular, lungs clear Abdomen: NAD - no pain or abnormalities on palpation Skin: pododermatitis on underside of RH paw but O thinks is from when slipped down stairs a week or so ago NOT primary skin disease Musculoskeletal: gait normal BCS: 5/9 Ddx: Otitis externa with possible allergies ASSESSMENTS: Ear cytology - presence of cocci +++ and occasional rod-shaped bacteria, mixed up ear smears so not sure which side had main infection TREATMENTS OPTIONS: - Daily ear cleaning with Otoflush Cleaner for rods - Use of oral steroids to reduce inflammation - Topical treatments BID a day to target cocci PLAN: - Initiate treatment with Otoflush daily to remove crusty material and alter pH in the ear canal. - Administer PMP medicated ear drops targeting cocci predominately. - Recheck and swab in 10 days to ensure resolution of the infection. - Owner instructed on the use of an adapter for easier administration of ear drops and to ensure accurate dosing. 20/02/2026 STAFF MEMBER:Elf REASON FOR CALL: scheduled callback OWNERS UPDATE/HISTORY: Ruby doing well, owner administering ear meds as directed, Ruby not exactly happy about it but is letting owner apply.
