Appointment Notes: vom and DH cfwHistory: Started saturday the vomiting and diarrhoea, okay sunday and monday but started yesterday again. Vomited first then had diarrhoea. Owner can confirm true vomit through abdominal contractions, salivation prior. Hunched up and trying to strain after had bout of diarrhoea. Saturday had some dark red blood in diarrhoea and small bit of mucous. Currently on low fat hills digestive, been getting chicken and rice since saturday. Still good appetite, still drinking okay.UTD vaccination and wormingNot on any medications, last dose of meloxicam was given a few weeks ago. Has had vomiting and diarrhoea bouts in past, usually self-resolve and has been discussed here prior. Not known to scavenege or eat anything she shouldn't. Examine: HR 132bpm, normal rhythmRR 24bpmEars, eyes, nose WNLMarked plaque build up and gingivitis, smelly breathMM pink, slightly tacky, CRT < 2secondsTemp 38.7Abdomen comfortable on palpation, hypermotile GIT soundsRectal exam empty colon but moist, anal glands very full LN palpable WNLAssessment: Acute gastroenteritis most likely. Highly unlikely to be FB/parasitic due to patient history. Possible some underlying IBD due to chronicity of vomiting and diarrhoea occuring in past. Plan: Patient clinically stable, no emergent requirement for hospitalisation or imaging today. Discussed with owner most likely going to self resolve, continue to feed bland chicken and rice for few more days. Pro-kolin paste to help firm up faeces, advised it won't treat the cause of diarrhoea only going to help with bulking up to reduce fluid losses.Owner has requested WAVES referral for workup as this has occured multiple times and has apparently discussed with Dr Brett already.
