04/03/2026 Diagnosis:Hepatocellular carcinoma, multi-focal/ nodular. Diagnosed Nov 2024; on Palladia, surgery for partial liver tumour resection on 17/06/25. Palladia revisit History: Physical Examination findings: Bright, alert and responsive. Heart & Lungs: no notable murmurs or arrythmias auscultated; normal respiratory rate and effort, normal bronchovesicular sounds on auscultation Gastrointestinal: soft comfortable abdomen on palpation Rectal exam:soft stools on glove, no internal lymphadenopathy Integument: no obvious lesions, wounds or ectoparasites Lymph Nodes: no peripheral lymphadenopathy noted Mm pink, crt < 2 secs, HR 102, RR 40 , Temp 38.8 , Wt: 3.4 Laboratory results: Standard profile + UA pending Treatment: Toceranib Phosphate 10mg PO MWF Comments: We will see Zorro again in x weeks time.
