<html><meta http-equiv="Content-Type" content="text/html; charset=utf-8"><p>Reason: Breathing funny?<br><br>Hx: Presented by Clare<br>Archie had his CT scan and results came back on the 2/4 - it was found that he had an enlarged LA with a small pulmonary nodule. Next step would be echo.<br>Since then O feels that Archie has been breathing a bit more rapidly but has counting his RRR and its been 22-24. <br>He did a small cough this morning as well but not as long as he usually does. O has got the dehumidifier going in the house so unsure if that's affecting Archie's coughing.<br>Is still eating and drinking normally, no v/d. Maybe a bit sluggish to get out of bed in the mornings but it has been colder the last few days.<br>No more obvious lameness/twitching on LHL.<br>Current meds:<br>1) meloxicam 6kg dose PO SID<br>2) 50mg gaba BID PO- O would like to see if we can reduce it. <br><br>Ex:<br>BAR, nice cat<br>Mm p crt <2s <br>Teeth G1<br>HR 184- no murmur auscultated<br>RR 32- normal breathing pattern noted, no obvious crackles or wheezes noted<br>Integument good<br>LNs NAD<br>Abdo palp NAD<br>BCS 7/9- stable<br><br>Assessement:<br>Enlarged LA on CT scan<br>3mm pulmonary nodule <br><br>Plan:<br>Booked in for echo + chest rads next friday- pending that, will know more about prognosis<br><br>Communication:<br>discussed with O to count his RRR daily when he's totally asleep- and we're looking for a trend.<br>So if he's consistently in the 30s, then let us know. It has been a bit humid the last few days so that could increase his RRR. Otherwise, he's still EDDU, no v/d- okay to wait for echo next fri.<br>With regards to back leg- suspect ST injury of some sort- ok to continue on meloxicam, okay to try and reduce gabapentin to 40mg BID PO for a week and if going well, then reduce to 30mg BID PO.</p></html>

Coughing fit this morning, usually intermittent coughing that has sometimes lined up with asthma.Breathing has been quicker with some effort on and off. Eating well.

2.40pm: STT called and LMOM for O to call back.O called back soon after.Essentially no evidence of any LHL issues- could be ST injury/sprain.But the more concerning thing is that his LA is markedly enlarged- so will need an Echo.O can go straight back to ARC to have echo done.There is also a small lung nodule (approx 3mm) that could be benign v malignant.Ideally to re-CT in 2-3 mths and if its bigger, to try and sample it.O will organise to have his echo done ASAP.have cancelled appointment with us on friday, O to re book after echo so we can discuss legs + heart at the same time.Assessment:There are no significant orthopaedic changes to the hips, stifle or spine to explain Archie's clinical signsof pain. A soft tissue injury is most likely (eg ligamentous tear or tendinopathy).A few concerning incidental findings were present, not relatable to the presenting complaint:- The left atrium was markedly enlarged, signalling that occult cardiac disease is likely- A single pulmonary nodule is present within the caudal right caudal lung lobe (approx 3mm indiameter). This may be secondary to a granuloma, haematoma, abscess or early neoplasia.

Hi Stacy,Archies CT scan went smoothly yesterday and he recovered very well! Eating and toileting as usual, hes quite alert and comfortable today after the sedation. ARC also said he woke up very well after.Ive rescheduled his appointment for tomorrow to next Friday afternoon as he is doing well. This should also hopefully be when the CT scan report has come back.Archie had his usual dose of metacam last night and started back on gabapentin this morning with no issue. I plan to keep him on the gabapentin and metacam combination until his consult next week - is it safe for him to continue the metacam for another week?If there is anything else I am concerned about with Archie I will be sure to let the clinic know, but for now I am very happy with how he is going.Thanks again for your help, have a nice weekend!Kind regards,Clare McKenzieM:0488 595 324_______________Hi Clare,It's lovely to hear that Archie has done well after his CT scan!His kidney parameters were very normal when we did his blood test last week so yes, he will be fine to continue on meloxicam for another week until I see you next Friday.
