Reason: Skin Rash Spreading From Haunches To Ears For 1WkAppointment Notes: Overdue remindersPresented by male owner Hx: O reports skin fine up until 7 days ago. Noticed lesions on bilaterally on forelimbs (dorsal to elbow), around the lip/muzzle, and bilaterally on base of pinna. Malaseb wash 2x over the weekNo history of allergies previously Often moving between houses - FIFOHangs around in sand in backyardED - normalUF - normalStools formedItching ears and arm pits. For ~1 weekDiet: PRime 100 roll, kibble FTW - not sure Meds: Previously stilboestrol: stopped and incontinence recurred, started again 4 weeks ago, incontinence improved. Has enough at home. PE: *** aggressive, could not examine. Reactive.Distance exam:Bilateral erythematous lichenified crusting lesions bilaterally above elbows. Erythematous inflammed lip foldsBilateral external ear canals erythematous, with crusting and ceruminous discharge. Ear bases red raw from scratching.OTOSCOPY COULD NOT BE PERFORMED Diagnostics:- bilateral ear cytology (++ cocci bilaterally)- impression smear (+ cocci)Assessment:Acute dermatitisBilateral bacterial otitis externaClinical signs suggest chronic (lichenification)- but O has only noticed ~1weekEnvironmental vs food vs ectoparasite vs otherPlan:- medicated shampoo (malaseb) - has at home. Wash 2x/week. Care to not get moisture near ears.- prednisolone 2 wk tapering course as per label. 1 tab for 2 days, 1/2 tab for 5 days, 1/2 tab every other day 6 days- cephalexin 750mg (3/4 tab) twice daily until finished (7 days - ACTUASLLY 21 TABS BISPENSED, NOT 10.5, DID NOT SAVE CORRECTLY)- easotic - bilaterally 1 pump per ear for 5 days. Explained administration but could not demonstrate on bonnie- Simparica trio sample given to start today - 20-40kg DO NOT CONTINUE EAR MEDICATION IN THE EVENT OF ANY HEAD TILT, WOBBLINESS OR ANY OTHER NEUROLOGICAL SIGNSO to contact clinic immedicately if these occur. O consented to start ear meds despite not safe if TM rupture. Revisit in 2 weeks**** attempted contact with female owner ~6pm 3/3/26 regarding confusion over medication regime, left VM to give call back from ~8am 4/3/26 for clarification - EJ Vital Signs Weight: 35;
