Appointment Reason: Check HipS:  o has notice Toby has always been reluctant to jump, doesn't want to walk on polished floors, reluctant to go up stairs, never jumps up on couch, goes for walk every days with dog walker for about 1hr, not very active dog otherwise, dog walker notice seems a bit stiff in one hind leg at end of walksO:  FAS 2/5 - nervous but nice, did not want to walk in to consult but once finished was ok to walk on floors in clinic, pink mm, grd 1 tartar, eyes appear normal dilated as stress but did constrict with light, lens clear, fundus appear ok so vision seems ok,  no neck pain, frontlegs appear ok,  chest abdo nad, cps normal, has some hair between pads, stifles nad, no mlp, no ADS noted r hip appears all fine, l hip slight resentment on full extension, no lumbosacral spine pain noted on flexion, psoas minor nadA:  ddx:  query l hip discomfort vs behavioural P:  recommend screening radiographs and palpate under sedation - if all normal consider behavioural medsest $715 for sedate and rads of lower spine/hips/hindlegs    Vital Signs    Weight: 13.2;
