Presented for: Last night refusing food and treat, blood in vomit this morning. Had occasional vomiting episodes after recent ingestion of children neurofen
Specs of blood in v+ 
Was ok since ingestion last Tues - maybe bit more thirsty 
3d after presenting at BVC he v+ again then stopped 
Stools have been on darker side but solid 
Urinating normal amounts or maybe less ? 
Yesterday evening off food, same this am 
No recent change in diet, does eat table scraps when baby throws off plate, not a scavenger
Does tend to chew toys not seen to swallow it 
Meds finished yest morning 
F&W: utd 

EXAM:
BAR, very excited 
HR 160bpm, no m/a, ssfp
RR panting, lung sounds clear 
Temp 39.2
Abdomen sl tense, not obv painful
MM pink, sl tacky, crt<2sec

ASSESSMENT:
Vomiting - ddx GI vs extra-GI
Recent hx of neurofen ingestion

HISTORY: 30 mins ago ingested 300mg of childrens neurofen. Consent for emesis

gastrointestinal effects

Dosage for gi effects = 75
Dosage for renal effects = 125
Dosage for CNS effects = 400

TPR wnl 

ASSESSMENT:
neurofen ingestion - Gi effects possible

PLAN:
apomorphien sq bw scap 
vomitus contained wrappers and crushed orange tablet
home with following:
losec quarter tablet twice daily and carafate 250mg twice daily to prevent ulcer

COMMUNICATION:
success in decomtamination so should be fine, only risk of GI symptoms, so home with ant-acids to prevent ulcer . call if concerned
