Reason:  6th amphotericin B Subjective:The interscapular swelling has all but resolved. 3x4cm swelling in R inguinal region still (following tx 5d ago).  Current meds = - ampB 0.8mg/kg SC last given Monday- fluconazole 150mg po BID- theophylline 300mg PO SID (given last night)Objective:HR/PR:  112bpm, normal pulses, sinus arrhythmiaTemp: 38.5MM: Pink, 2s crtPulse:  normalResp: 20bpm, normal sounds and pattern. Not congested or stertorous on cxxHydration: nad Assessment: OK for tx. Treatment: AmpB 0.8mg/kg -> 12.5mg or 2.5mlsInjected into NaCl 0.5%+ 2.5% glucose,  warmed to 40 degrees (achieved this by heating the 500ml bag in microwave for 60s).  Given via extension line injected with 22G needle under skin R and L flank fold.Infused entire bag in <20mins.Site massaged post-infusion. Plan: - rv Friday for 4th AmpB rx (0.8mg/kg)- rv Monday for 5th  AmpB rx (0.8mg/kg) + rpt bloods- continue fluconazole 150mg BID PO - continue theopylline 300mg SID PO     Vital Signs    Weight: 15.5;
